WORKSAFE WA says it is investigating the death of a 50-year-old roof plumber in south Perth on 26 August 2009.
The man was believed to have been working on the roof of a four-storey building on South Perth Esplanade when he fell to the ground.
WorkSafe WA has sent its inspectors to the site and will investigate the circumstances.
