WORKSAFE WA is investigating the death of a 38-year-old machinist at a metal manufacturing company in Canning Vale on the evening of 11 August 2010.
According to WorkSafe WA, the man was believed to have been operating a vertical boring machine when part of the machine fell, crushing him.
Inspectors attended the site and will interview witnesses and investigate the circumstances.
